Care Coordinator

This role is open to both part-time and full-time applicants. Essential qualifications include proven experience in conducting Child and Adolescent Needs and Strengths (CANS) assessments and developing Plans of Care (POC). Candidates must also demonstrate proficiency in Family Centered Meeting (FCM) methodologies, possess superior communication abilities, and exhibit meticulous attention to detail.
